HCTN Referral Bonus Program – Terms and Conditions
These Terms and Conditions govern the Referral Bonus Program (the “Program”) offered by Health Carousel Travel Network, LLC (“HCTN”)and effective as of March 17, 2025 (the “Effective Date”). By participating in the Program, employees agree to comply with and be bound by these terms and conditions.
Eligibility
To be eligible for a Referral Bonus, the referring individual (“Referrer”) may not be an internal, corporate employee of HCTN or any HCTN affiliate, or a family member of an internal, corporate employee of HCTN or any HCTN affiliate. The referred individual (“Referee”) must:
a. Not have previously been employed by, or accepted employment from, HCTN;
b. Not have spoken with or submitted documentation to HCTN in the twelve (12) months immediately preceding thereferral (this includes submission and/or communication via any means including, but not limited to, email, text, website form submission, or On Demandportal); and
c. Successfully complete at least two (2)weeks of work on an HCTN assignment.
Referral Bonus Amount
The Referrer will receive a Referral Bonus of one thousand dollars ($1,000) for each individual they refer who is hired by HCTN and successfully completes at least two (2) weeks of work on assignment.
Payment of Referral Bonus
The Referral Bonus will be paid to the Referrer no later than the seventh (7th) week following the Referee’s assignment start date. The payment will be subject to all applicable federal, state, and local tax withholdings, as required by law.
Program Modifications and Termination
HCTN reserves the right to modify, amend, or terminate the Referral Bonus Program at any time, in its sole discretion, without notice to employees. Any such changes will be effective as of the date specified by HCTN.
Miscellaneous
a. A candidate may only be referred to HCTN once. The first eligible Referrer for such eligible candidate will receive the referral if all other requirements are met.
b. This Program cannot be combined with any other referral or hiring bonuses offered by HCTN.
c. HCTN is not responsible for any in corrector incomplete information provided during the referral process.
d. In case of any disputes or issues relating to the Referral Bonus Program, HCTN's decision will be final.
By participating in the Program, the Referrer acknowledges that they have read, understood, and agree to abide by these terms and conditions.
